Comes in a double sided, fold-over cardstock cover sleeve with credits and thanks list printed inside.
Also comes with 2 double sided, 14" x 7" inserts printed on standard paper that fold together to create what is essentially a 12 panel cover / lyric booklet when all put together.

Recorded September 1996 at Smegma Studios.
Catalog number "CON 011" appears on the center label and "con eleven" appears on the back of the cover sleeve.
